A select team of Hospice at Home caregivers brings end-of-life care to people living in long-term care facilities.
As specialists in facilities-based care, our Facilities Team focuses on seamless and consistent collaboration between Montgomery Hospice and the facility.
Benefits of facility-hospice partnerships include:
- Extra "hands and eyes" on the institution's sickest patients, freeing facility staff to spend time with other patients
- Fewer hospitalizations, higher patient/family satisfaction
- Better performance on quality surveys (Pain and symptom management is part of JCAHO, CMS and state surveyors' evaluations of facilities.)
- The Medicare Hospice Benefit prescribes three levels of care within facilities: Routine, Continuous and Respite.
Billing care of hospice patients who live in facilities generally works the same as for other Hospice at Home patients. Montgomery Hospice is responsible for billing Medicaid for care of Medicaid patients on hospice in facilities.
