How to Apply

We ask prospective adult volunteers to provide:

• Biography/Statement of Interest—This form can be completed online, or you may print a copy (PDF 172KB) and return it to us by US Mail
• Two non-family References (PDF 172KB)—These should be printed, completed and returned to us by US mail
• Physician's Statement (PDF 188KB)—necessary only for applicants who want to volunteer in patient care; should be printed and returned to us by US mail
• An in-person interview at our offices with someone from our Volunteer Services staff

Mailed materials, including two references and the physician's statement for prospective patient care volunteers, should be sent to:
