How to Apply

If you are interested in becoming a Montgomery Hospice volunteer, call the Volunteer Services office at 301-921-4400. One of the volunteer managers can tell you about upcoming trainings and current volunteer opportunities. Then return to this page for the necessary forms. Prospective adult volunteers will be asked to provide:

• Biography/Statement of Interest—This form can be completed online, or you may print a copy (PDF 172KB) and return it to us by US Mail
• Two non-family References (PDF 172KB)—These should be printed, completed and returned to us by US mail
• Physician's Statement (PDF 188KB)—necessary only for applicants who want to volunteer in patient care; should be printed and returned to us by US mail
• An in-person interview at our offices with someone from our Volunteer Services staff
